Is a COVID-19 crisis looming for Japan?
Le 3 janvier 2020, un groupe de cas de pneumonie grave d’étiologie inconnue ou de cause inconnue dans la ville de Wuhan, province du Hubei, a été signalé par le ministère chinois de la Santé. En réponse à cette maladie infectieuse émergente de cause inconnue, le Japon a mis à jour son système de surveillance des maladies le 6 janvier 2020 afin de capturer les cas suspects de pneumonie liés à l’épidémie originaire de la ville de Wuhan. Cela a été fait conformément à la loi: « Loi sur la prévention des maladies infectieuses et des soins médicaux pour les patients atteints de maladies infectieuses », article 14, paragraphe 1. La cause des cas de pneumonie sévère originaires de la ville de Wuhan, en Chine, a été identifiée comme un nouveau coronavirus (2019 -nCoV) et est devenue une maladie infectieuse désignée au Japon le 1er février 2020. Ce document résume les 12 cas confirmés de 2019-nCoV détectés avant le 1er février 2020 avant l’ajout de 2019-nCoV en tant que maladie infectieuse désignée. Ces informations ont été recueillies auprès des municipalités concernées par le ministère de la Santé, du Travail et des Affaires sociales, le 3 février. À l’avenir, certaines des informations suivantes pourraient être révisées ou mises à jour.
Sont exclus de ce résumé les huit cas détectés parmi des citoyens japonais qui sont retournés au Japon sur des vols charters au départ de Wuhan le 31 janvier, car ces cas sont toujours sous surveillance (au 3 février 2020).
Du 15 au 31 janvier 2020, un total de 12 patients ont été confirmés en laboratoire comme cas de 2019-nCoV (au 3 février 2020). Les tests génétiques des spécimens ont été effectués à l’Institut national des maladies infectieuses (NIID) ou aux instituts de santé publique locaux des municipalités concernées. La répartition par âge des cas variait de vingt à soixante-neuf ans, avec 2 cas dans la vingtaine, 3 cas dans la trentaine, 5 cas dans la quarantaine, 1 cas dans la cinquantaine et 1 cas dans la soixantaine. La répartition par sexe était de 50:50 avec 6 hommes et 6 femmes. La date d’apparition des symptômes des cas allait du 3 au 26 janvier 2020. (Figure.1)
Sur les 12 cas confirmés, six ont été importés de Chine, cinq directement de la ville de Wuhan et un de la province du Hubei autre que la ville de Wuhan. Sur les six cas restants, il y en avait un dans chacune des préfectures suivantes: Kanagawa, Nara, Osaka, Mie, Kyoto et Chiba.

Neuf des 12 cas avaient des antécédents de voyage ou de séjour à Wuhan dans les quatorze jours suivant l’apparition des symptômes, les trois autres cas n’ayant pas d’antécédents récents de voyage en Chine. Au 3 février à 12 heures, les conditions des 12 cas s’étaient améliorées ou devenaient stables. La recherche des contacts et l’observation des contacts étroits ont été achevées pour un cas le 24 janvier 2020.

Feel sorry for the people on the ship but also Japan. Bear in mind they were the 1st country outside China to have to really deal with this and they weren’t prepared (a lot of information about this virus still not 100% known). Foreign gov should send chartered planes to pick up all the foreigners on board. Japan has no facilities to house 3700 onshore. And the Japanese onshore start showing the symptoms after large amount of Chinese visited Japan during Chinese holiday. So the hospital has to be ready for those Japanese. I only heard the report about infected people is rising everyday at each different town in Japan, I hope Japanese Government will do something seriously to prevent the people against the virus, they should educate their people to realize that how this virus is dangerous plus the atmosphere in Japan seems to be spreading easily especially in crowded area ; public transportation, highest proportion of elderly citizens that may get weak immune system.

Not sure why everybody thinks the passengers were infected while under quarantine when we know that the incubation is up to 2 weeks (maybe longer) and there was significant time between patient zero and day 1 of quarantine. « Tested positive » today means you were infected 2 weeks ago.
If this virus came from a province in Canada, people would want those people to isolate and not travel to other countries either. It has nothing to do with ethnicity and everything to with the location the outbreak started, people from different provinces inside of China are even discriminating against people from Hubei as well. If the Canadian government stopped flights from China, people in Canada wouldn’t be so worried that someone just flew here from the epicenter. Canada failed. i was thinking why cant Japan have 30 Mobile tents built, with 100 passengers each. Test the passengers and clear them, they need to separate the sick and healthy.
China built a hospital with 1000 beds in 10 days, but Japan made the 3000 passengers stay on the ship for 2 weeks, because they didnt have any space to house them.

船下，各部门密集布防。“海巡0203”轮在邮轮锚泊船水域值守应急、全程护航，确保一旦险情解除，可在最短时间内让邮轮安全进港靠泊。

直升机从塘沽机场升空，进行第一次勘察飞行。绕飞“歌诗达赛琳娜号”，空中察看船舶和海面情况，机长判断：各项情况良好，可以执行空中接力送样。

直升机到达邮轮海域上空，盘旋巡视后，飞向临近的“广东龙”货船甲板，等待最后指令。

直升机接令第三次升空。3分钟后，抵达“歌诗达赛琳娜号”船尾左舷绞车点处，在甲板六七米上方悬停。放置了17份检测样本的黄色标本箱，以悬吊方式缓缓升空、进入机舱，直升机向海岸飞离。

直升机返回塘沽机场，工作人员跑上前接过标本箱、送上转运车，由警车开道护送，一路飞驰而去。

检测样本送达天津市疾控中心。
（15点左右）
17份标本检验结果全部为阴性，排除新型冠状病毒感染肺炎。

游客开始陆续下船，现场秩序良好。

3706名游客全部下船。下船后，600余名游客由旅行社安排离开，1500余名游客乘坐新区提供的大巴车分别前往轻轨站和火车站，1600多名游客由家人、朋友接走。
35名湖北籍游客由于疫情影响暂时不能返乡，滨海新区卫生健康委、公安局、交通运输局等部门分工协作，将35名湖北籍游客顺利安置到新区政府招待所。并为每个房间配备了体温计，每天早上8点和下午4点监测游客体温。
歌诗达公司为按照原计划将在25日登船旅行的游客全部取消航程并实现退票。

零时15分，天津官方宣布，“歌诗达赛琳娜号”邮轮应急处置工作全部结束，此时距第一道命令发出刚好近24小时。
Why are Italy and South Korea hit so hard?
Short answer: Because of the quality of its health care system, the outbreak was detected. Undetected outbreaks in many other countries are very likely.
Long answer: COVID-19 is a dangerous pandemic that will possibly infect 50 percent of the world population.
The reason that so few people realise this, is that the main indication for a COVID-19 pandemic is not what we currently see (decreasing numbers of infections in China, small outbreaks in Italy and South Korea), but the main indicator for the upcoming world wide outbreak is what we currently do not see.
What do we currently see? Outbreaks in Italy and South Korea. The interesting “coincidence” about the recent reports on the outbreak in South Korea and Italy is that both countries have a high level health care system and a high level of preparedness for such an outbreak.
There are hundreds of countries in the world where an outbreak is possible since the virus already has spread to all continents except South America and Antarctica. But we now see well documented outbreaks in two specific countries: South Korea and Italy. Both countries have a high level of healthcare and a high level of preparedness for a virus outbreak. Logically, there are three explanations for this coincidence:
The COVID-19 disease chooses countries with high level of health care and preparedness (obviously not true).
It is just a coincidence.
There are currently many cities and villages around the world with an outbreak, but outbreaks are only detected and followed up at locations with a high level of healthcare and preparedness.
I think option 3 is the obvious explanation. Which implies that as we speak, many many outbreaks like in South Korea are happening in countries like Myanmar, North Korea, India and Iran. These outbreaks will go undetected, and adequate measurement like large scale testing of the local population will not be done. And subsequently there are no reports in increasing numbers of infections.
An indication of such hidden outbreaks and thus a pandemic scenario is that infections will be found in people that have not visited China, and have not been in contact with people who did, but who did travel in countries where no large scale outbreak has been reported.
Exactly that just happened: the latest infection found in Canada. That infected person had been to Iran, where according to the official figures less than 30 people are infected, in a population of 50 million. It is highly unlikely that the Canadian is infected in Iran by having contact with one of these less than 30 infected people. It is far more likely that already many (ten thousands?) people in Iran are infected, but not tested and not reported.
In the coming weeks we may see more and more of these improbable traveller infections, which will create further evidence of the pandemic spread of the virus. And yes, such a pandemic is uncontainable, just like the flu. The US nor any other country is capable of keeping the flu virus out. COVID-19 is more contagious than the flu. So COVID-19 will spread throughout the world and you will likely be exposed to the virus some day later this year, unless you flee to a secluded cabin and live self-sustained.

What is also is distressing, is that for each person that has so far deceased from the virus, not 49 people are cured (2 percent fatality rate), but only 9. Which suggests that the virus actually has a 10 percent fatality rate. The theory that the fatality rate is just 2 percent (total number of deceased people, divided by the total number of infected people) Is based on the obviously flawed assumption that all currently infested people will be cured.
Bases on the observations above, I expect there will soon be concern and panic, and the world economy will probably crash as a result.

Australian government issued advice earlier this week on how international students in China « may » be able to get back to Australia via a third country and provided examples, as Australia currently has a ban on most travellers from China.
Some universities are actually funding students to take this approach.
